- 2 passport-sized coloured photos (4.5cm × 3.5cm)
- Duly filled Form G
- Coloured copy of a valid ID issued in Korea (residence / alien card) for the applying parent(s)
- Birth certificate of the child showing mother and father
- English translation of the birth certificate if it is in another language
- Coloured copies of both parents’ passports (for Ugandan passports: visa and bio-data page + page 45 where applicable)
- Coloured copies of both parents’ valid Korea-issued IDs (residence / alien cards)
- Return envelope with postage for registered mail if posting
